Please read this document carefully before signing. By signing below, you agree to the assumption of risk, release and waiver of liability, indemnification, and photo/video model release set out in this form in order to participate in the strength competition and other activities at the booth hosted by The Strength Factory and Legacy Lifestyle Company (together, the 'Companies') at the Sample Triad event.

SECTION 1 — ASSUMPTION OF RISK

I understand that participation in a strength competition and related event activities involves physical exertion and the use of weights and exercise equipment, and carries inherent risks that cannot be eliminated regardless of the care taken. Those risks include, but are not limited to, muscle strains and sprains, joint and back injuries, falls, contact with equipment or other participants, aggravation of pre-existing conditions, serious injury, and, in rare cases, permanent disability or death. Risks may also arise from the condition of the premises, equipment, and the actions of other participants, spectators, and event staff. I confirm that my participation is voluntary, that I am physically able to take part safely, and that I will follow all instructions from booth staff, use equipment only as directed, and stop participating if I feel unwell or unsafe. I knowingly and freely assume all risks, both known and unknown, arising from my participation, even if arising from the negligence of the Companies or others, to the fullest extent permitted by law.

SECTION 2 — RELEASE, WAIVER OF LIABILITY, AND INDEMNIFICATION

In consideration of being permitted to participate, I, for myself and for my heirs, executors, administrators, personal representatives, and assigns, release, waive, discharge, and covenant not to sue The Strength Factory and Legacy Lifestyle Company, and their respective owners, officers, members, managers, employees, coaches, staff, volunteers, agents, sponsors, and representatives (together, the 'Released Parties'), from any and all liability, claims, demands, actions, damages, costs, and expenses of any kind arising out of or related to any loss, damage, or injury, including death, that I may sustain while participating in or traveling to or from the event activities, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by the laws of the State of Oklahoma. I further agree to indemnify, defend, and hold harmless the Released Parties from any loss, liability, damage, claim, or cost, including reasonable attorneys' fees, arising from my participation, my breach of this agreement, or any claim brought by or on behalf of me or anyone claiming through me. This release does not apply to claims arising from gross negligence or willful misconduct where such a release is prohibited by law.

SECTION 3 — PHOTO / VIDEO MODEL RELEASE

I understand that the Companies will photograph, film, and record participants at the event. I grant The Strength Factory and Legacy Lifestyle Company, and their assigns, licensees, and successors, the irrevocable, perpetual, worldwide, royalty-free right to photograph, film, record, reproduce, edit, publish, and use my image, likeness, appearance, name, and voice as captured at the event, in whole or in part, in any medium now known or later developed. This includes use in advertising, marketing, promotion, social media, websites, video content, and other commercial and promotional material for either Company. I understand that I will receive no compensation for any such use, that I have no right to inspect or approve the finished material, and that the Companies own all rights to the recordings they create. I release the Companies from any claim arising from the use of my image, likeness, or voice as permitted by this section, including claims for invasion of privacy or right of publicity, to the extent permitted by law. If I do not wish to be recorded, I will tell booth staff before participating. I understand the strength competition may be recorded as part of the event.

SECTION 4 — SMELLING SALTS SAMPLING — WARNINGS AND ACKNOWLEDGMENT

Legacy Lifestyle Company may offer samples of Legacy Smelling Salts at the booth. Sampling is entirely voluntary. You do not have to sample the product to take part in other event activities, and you may decline or stop sampling at any time. Please read these warnings carefully before you sample. If you choose to sample, you must initial this section below. WARNING: This product releases ammonia vapor and can be hazardous if misused. It is for brief inhalation only, exactly as demonstrated by booth staff. Use only as directed: Sniff lightly from a distance, as demonstrated by staff. Do not inhale deeply, do not hold the bottle close to your nose, and do not take repeated sniffs. Possible effects: The vapor may irritate the eyes, nose, throat, and lungs, and may cause coughing, sneezing, watery eyes, headache, nausea, or dizziness. Keep the product away from your eyes and avoid skin contact. Do not ingest the product; it is not for consumption. Do NOT sample if: you have asthma or any respiratory condition; you have an allergy to ammonia or ammonium compounds; you have a heart or blood-pressure condition; you are pregnant; or you have any other medical condition that could be aggravated by inhaling ammonia vapor. If any of these apply, or if you are unsure, consult a physician before sampling. Booth staff may decline sampling to anyone, for any reason. Not for children: No sampling by minors (under 18), even with a parent or guardian present or with a signed form. Keep the product out of reach of children at all times. If you feel unwell: Stop immediately and move to fresh air. Seek medical attention for difficulty breathing, severe irritation, or symptoms that do not go away. First aid is available at the booth — tell booth staff right away if you need help. By initialing below, I acknowledge that I have read and understood the warnings in this section, that I am 18 years of age or older, and that I am sampling the product voluntarily and at my own risk. I understand that this acknowledgment, and any sampling I do, is covered by the assumption of risk, release, waiver of liability, and indemnification in Sections 1 and 2 of this form, which apply to sampling just as they apply to the other event activities. Initials are required to sample Legacy Smelling Salts. No initials, no sampling. Leave blank if you are not sampling.

SECTION 5 — GENERAL TERMS

Health: I confirm I have no medical condition that would make participation unsafe, or I have consulted a physician and accept the risk of taking part. If I am injured, I consent to basic first aid and emergency assistance being sought on my behalf. Conduct: I will follow event and venue rules and staff instructions. Staff may stop my participation at any time for safety or conduct reasons. Entire agreement; governing law: This form is the entire agreement for my participation at this event. If any part is held unenforceable, the rest continues in full effect. This agreement is governed by Oklahoma law, is intended to be as broad as Oklahoma law permits, and binds me and my heirs and assigns. Do not sign until you have read the whole form. By signing, you acknowledge that you understand it, agree to all of its terms, are giving up substantial legal rights, including the right to sue the Companies for ordinary negligence, and are granting the Companies the right to use your image and likeness in advertising without compensation.
